Easily register online as a new patient

We would also like to receive a copy of the medical questionnaire completed by you from adults (18+). You can take this with you on your first visit to the practice.

After receiving your online registration, you will be contacted for an intake and introductory consultation.
During this consultation, the following topics will be discussed, among others:

• your completed medical questionnaire
• your dental background
• complaints and experiences in the past
• the oral examination
• recording the dental status and gum condition
• report of the findings of the investigation

Depending on the findings and wishes, a follow-up appointment will then be scheduled for treatment or further consultation.
It is not usual to provide extensive treatment on the first visit. We attach great importance to making a good start to the treatment relationship.
